Repeat Prescription Request Form
Please complete the form below and allow at least 24 hours once this form has been submitted before collecting the medication from the clinic. This allows a veterinarian to review your pet’s records and confirm the medication is ok to dispense.
Your pet must be a current patient of the clinic and has been seen within the last 6 months for an examination by a veterinarian.
There are some medications that cannot be dispensed without a current or recent consultation – this is at the discretion of the attending veterinarian.
